It is DAMAGE inflicted. So your ATK, enemy def, modifiers and whatnot also applies.

Your Damage = 70
1% of 70 = 7 HP regained

Simple ne?
Raise your ATK and other damage modifers to increase your HP regain

Yes it stacks: however, you'll only regain 1% from each attack, with the Solar Sword in the offhand getting a penalty.

It does not work on magic, only Physical Damage. It may work with skills, I haven't tested that.

Although, if you're going to go for Health Regeneration from Damage, I think it'd be best to do a Triple Malicious (Hunter Fly) Saber with a Bazerald in the offhand. You'd get more attack, (115 versus 85) and you'd get a better chance of absorbing more damage (1% on each attack versus 9% chance to get 15% of damage) and, with the bazerald (this is optional) you'd have the fire element.
